What is a trigger point?

A myofascial trigger point (or trigger point) is a hyperirritable area in a skeletal muscle — a tight band of contracted muscle fibers that can no longer relax completely. These points have two distinctive characteristics: they are painful to local pressure, and they reproduce referred pain in an area away from the point itself — this is called the referred pain pattern.

For example, a trigger point in the suboccipital muscle can cause headaches. A stitch in the piriformis can mimic sciatica. A stitch in the infraspinatus muscle can cause shoulder pain felt like bursopathy. This is why so much chronic pain is resistant to conventional treatments — we treat the painful area, not the source.

How is a trigger point treated?

The trigger point release technique involves applying sustained, gradual pressure directly to the point for 30 to 90 seconds — until you feel a reduction in local and referred pain. This ischemic pressure forces contracted muscle fibers to relax, interrupting the pain-contraction-pain cycle.

Additional techniques include post-isometric stretching, transverse friction, assisted active mobilizations and in some cases dry needling — practiced only by therapists who have the appropriate training.
